Periapical lesions can be of endodontic or nonendodontic origin. Especially when nonendodontic periapical lesions are associated with teeth with pulp necrosis or that were previously treated endodontically, it may lead to misdiagnosis and an ineffective therapeutic protocol. This article summarizes the imaging diagnosis and differentiation of the periapical radiolucent lesions, and provides reference for clinical diagnosis and treatment.
