目的 收集锥形束CT(CBCT)资料,分别沿垂直方向和腭侧骨板方向测量上颌第一磨牙区牙槽骨高度和宽度,为上颌第一磨牙区种植体偏腭侧植入及治疗计划制定等提供解剖学依据。方法 选取南方医科大学口腔医院CBCT影像资料库中169例患者资料为研究对象,测量上颌第一磨牙缺失时缺牙区的垂直向牙槽骨高度和宽度、腭侧骨板方向的牙槽骨高度和宽度,以及上颌第一磨牙存在时的垂直向牙槽骨高度和宽度。每个项目反复测量3次,间隔2周后作第2次测量,6次测量数据取均值。结果 上颌第一磨牙牙槽骨平均高度9.33 mm,平均宽度13.28 mm。90例第一磨牙缺失的患者,缺牙区牙槽骨平均高度6.99 mm,平均宽度8.93 mm。垂直向牙槽骨高度不足5.0 mm者有28例,沿腭侧方向测量时,以3.5 mm为最小宽度,有10例(35.7%)可以获得比垂直方向更多的骨量;以4.0 mm为最小宽度,有9例(32.1%)可以获得比垂直方向更多的骨量;以4.5 mm为最小宽度,有6例(21.4%)可以获得比垂直方向更多的骨量。结论 上颌第一磨牙缺失时,存在部分病例沿腭侧方向比垂直方向能获得更多的骨量,可为上颌第一磨牙区种植体偏腭侧植入提供依据。

英文摘要

ObjectiveCollected cone-beam computer tomography(CBCT) data, to provide anatomia basis for clinical application of implants in the maxillary first molar area palatal side bone by measuring the maxillary first molar area alveolar bone height and width along the vertical direction and the palatal transverse plate.MethodsA total of 169 patients evaluated by CBCT scans were included in the date bank of Guangdong Provincial Stomatological Hospital were analyzed. Both height and width of the alveolar bone in the edentulous area of maxillary first molars and that of the alveolar bone of maxillary first molars were repeatedly measured three times every two weeks to get mean value.ResultsMean alveolar bone height and width of the first molar was respectively 9.33 mm and 13.28 mm. Mean alveolar bone height and width of 90 patients with first molar loss was respectively 6.99 mm and 8.93 mm. There were 28 patients with less than 5.0 mm height of alveolar bone. With 3.5 mm, 4.0 mm and 4.5 mm respectively defined as the minimum width, compared with vertical direction, more bone mass were respectively obtained in 10 patients(35.7%), 9 patients(32.1%) and 6 patients(21.4%) when measuring in lateral direction of the palate.ConclusionWith the maxillary first molar lost, more bone mass can be obtained in some patients along the lateral direction of the palate, which could provide basis for clinical application of implants in the maxillary first molar area palatal side bone.
